We did not anticipate correctly or prepare for a terrorist attack that resulted in the killing of a US Ambassador, something that has happened something like only 4 times in history. We were woefully unprepared for what happened and people died. The investigation just proved that there wasn't a coverup of the event or the lack of a proper response after the fact. It didn't disprove that we screwed up royally and some pretty high ranking people paid for it with their lives.
