I don't think people who appear female while having XY chromosomes are really biological females or that people who appear to be male while having XX chromosomes are really biologically male. I think they are biologically, as a functional matter, asexual. They are infertile. Both conditions are disorders. I did find a report of a "woman" with XY chromosomes having a successful pregnancy but it was with donor eggs and embryo transfer.

Both are rare. Something on the order of 1 in 20,000 apparent female births for XY "females" and 1 in 20,000 apparent male births for XX "males." Being a reproductively functional member of one sex or the other is indeed a binary thing determined by whether you have XX or XY chromosomes.
